Health-care Card
All foreign citizens resident in Italy with a regular residence permit have the right to insured health assistance from the National Health Service (SSN), and will be treated on equal terms with Italian citizens.
Health assistance and thus the Health Card is the document which bears witness to enrolment in the Health Service and allows one to use all health services
This assistance is extended to all family-members who have regular residence.

How to request
It is released by the ASL (local health office)and to obtain it it is necessary to present:
-a copy of the residence permit;
-a copy of the fiscal code;
-The certificate of residence or self-declared certification.
The Health Card is sent through the post and is valid for 5 years.
In case of loss or theft it is necessary to go to the ASL desk or to the Income Office and fill in a form to obtain a new Health Card.
Necessary Information
The Health Card states:
-personal particulars;
-fiscal code;
-health data reserved for the District Office;
-expiry date of Card.
The back of the Health Card is valid as European Isurance Card against illness (TEAM).
